- Client Information: Full name of the person we’re transporting.
- Appointment Details: Date and time of the appointment.
- Addresses: Both the pick-up and drop-off addresses.
- Personal Details: Date of birth and insurance information.
- Contact Information: Your case manager’s name (if applicable).
- Medicaid Details: Medicaid number, if you’re a Medicaid beneficiary and Primary Care Physician.
- Assistive Equipment: Please let us know if you’ll be bringing any assistive equipment such as a cane, crutches, oxygen, walker, or wheelchair.
- Companion Details: Please inform us if a family member, friend, aide, or service animal will be riding with you. If you’re bringing a service animal, please provide information about the animal’s certified training and whether it’s properly caged or restrained.
